The second part of the course is about sustainability, helping you stick with new habits and avoid slipping back into old ones. It's about ongoing progress and moving toward your goals. You will focus on enhancing the skills you’ve learned, and maintaining the changes you’ve made.
Your DPP nutritionist will continue with you during this stage, helping you make further tweaks as you progress.
Nutrisense believes in a sustainable approach. Learning new skills, creating habits, and building confidence takes time. Throughout our comprehensive 12-month program, you will focus on making practical and manageable adjustments.
Empowering you with the right tools to be successful
Track your data to keep yourself accountable, monitor your progress, and achieve your goals. But remember, you’re not alone. You will have your group's support and your DPP nutritionist will stay connected along the way.
Set manageable and realistic tasks
We understand the nuances of everyday struggles that may pose as obstacles toward achieving your goal. Together with your DPP nutritionist, you will come up with plans that fit into your day-to-day lifestyle.
Track your progress
Log your weight, A1C results, meals and activities. This helps you understand how your current approach impacts your progress. Your DPP nutritionist will guide you during group sessions to help you better understand how your habits are helping or hindering your progress.
Actionable feedback
You will be paired with a group of individuals with similar goals and challenges. With the guidance of your DPP nutritionist and the support of this group, you will learn how to connect the dots between your lifestyle and your health goals.
